Thirty-two patients, December 1999 to August 2001: endoscopic malar reduction from a single temporal incision, with two dissection planes and no intraoral cut.
What was recorded
Malar reduction was carried out through one incision inside the temporal hairline. From that incision two dissection planes were separated — one leading to the zygomatic arch, one to the zygomatic body — so that a complete osteotomy of the arch and an incomplete osteotomy of the body could be made under endoscopic view without opening the mouth. The series was deliberately narrow: 32 selected patients over twenty months.
What it shows
Within the published literature this is the earliest systematic report of a single-temporal-incision, two-plane endoscopic malarplasty; earlier reports had combined temporal and intraoral incisions, or used several scalp incisions with a laser. The paper marks the point at which the temporal passage, opened for the forehead in 1999, was carried down to the cheekbone.
What it does not show
The series began with selected patients and a narrow indication, and the paper says so. It does not claim the approach for every face: where a large movement and rigid fixation are needed, the intraoral approach may suit the face better. The 32 cases are not added to the later series of 11 and 261, which are separate studies.
One operation, recorded at three points in time — first systematised in 2003, extended to revision in 2013, extended in indication in 2021 — on the same temporal passage.
